Brio Living Services in Holland, MI is seeking a Social Worker (BSW) to join our LifeCircles PACE care team. You will empower participants to live their best lives by supporting emotional well‑being, guiding transitions, and connecting them to community resources.

Qualifications

• Must possess a BSW from a CSWE-accredited program.
• Active MI LBSW/LLBSW license.
• Minimum of one year of experience working with frail or elderly populations.
• Must possess a current, valid driver’s license and maintain auto insurance.
• Current CPR certification, or willingness to obtain within 6 months.

Responsibilities

• Case Management & Care Coordination: Manage and coordinate care plans with healthcare professionals, families, and community organizations.
• Facilitate participant transitions of care with compliant handoffs between care settings.
• Participant Support & Advocacy: Conduct wellness checks and connect participants to housing, healthcare, and financial resources.
• Screening & Eligibility: Assist in assessing financial eligibility and required documentation.
• Collaboration & Communication: Maintain ongoing communication with participants, families, facilities, and external agencies.
