Job summary

Garden Place Healthcare in Attleboro, MA is seeking a Licensed Social Worker (BSW) to support residents and families, develop stay plans, and coordinate discharge planning within the interdisciplinary team.

Qualifications

• Social Work Licensure.
• Long Term Experience: 1-3 years minimum.

Responsibilities

• Promote positive morale and work with residents, family and team to outline stay goals and discharge plans.
• Conduct psychosocial screenings and prepare initial plan of care including MDS paperwork.
• Discuss Resident Rights with new admissions and assist with admission agreements.
• Coordinate with families and Medicare/Medicaid for financial assistance and discharge planning.
• Schedule interdisciplinary team meetings to plan care and address concerns.
