Job summary

Brown University Health is seeking a Clinical Social Worker to assist patients and families in resolving medically-related problems by locating and coordinating resources across hospital, community, and financial services.

Under supervision of a Clinical Social Work Manager, you will develop service plans, maintain documentation, and coordinate referrals while upholding the organization’s values of compassion, accountability, respect, and excellence.

Qualifications

• Bachelor's Degree in Social Work from an CSWE-accredited program.
• At least one year of relevant healthcare or managed care experience.
• Strong analytical and interpersonal skills to work with patients, families and medical staff.

Responsibilities

• Consistently applies the organization's values in delivering high-quality, patient-centered social work services.
• Interviews patients and families to assess needs within family functioning.
• Provides information to hospital staff and community agencies to resolve issues.
• Develops and implements a service plan for patients and families.
• Maintains documentation and statistics of case activity.
• Participates in departmental studies and projects as needed.
• Maintains and updates lists of hospital and community resources.
• Pursues ongoing professional development to enhance expertise.
• Assists in daily living problem-solving, transportation, housing, and education needs.
• Coordinates referrals to appropriate community programs.
• Supports discharge planning and placement coordination with health care team.
• Engages in utilization management activities to secure authorizations and reviews.
