Job summary

Pressley-Ridge is seeking a Bridges Liaison in the Cleveland area to coordinate services for youth and families, ensuring housing, education, employment, and independence needs are addressed in a strengths-based, culturally sensitive approach.

You will work with a treatment/service team, maintain detailed records in SACWIS, and advocate for individuals to access natural and community supports. Flexibility and collaboration are key to driving positive outcomes.

Qualifications

  • Requires bachelor’s degree in human services.
  • Requires a valid state driver’s license, safe driving record, and current auto insurance.
  • Requires two (2) years of supervised experience with children and/or families.

Responsibilities

  • Provides professional coordination of services according to the individual’s level of need, working as part of the treatment/service team.
  • Develops relationships with the individual and important people in his/her life with consent of the individual served.
  • Assesses strengths and needs through interviews and reviews of social and clinical information.
  • Maintains an accurate and timely record of Bridges activities in SACWIS per required timeframes.
  • Contributes to the development and implementation of comprehensive service plans.
  • Advocates for the individual's needs and coordinates services to support housing, education, employment, and independence.

Position Summary

The Bridges Liaison provides Service Coordination carefully calibrated to the level of need of the individual being served. The Bridges Liaison works in a collaborative manner with the young adult to assure that the needs for housing, education, employment and independence are respectfully and adequately addressed. In each case the Bridges Liaison will assure that services to the young adults are individualized, strengths and needs based, culturally sensitive and responsive, and goal driven.

  • Clinical and Support
    • Provides professional coordination of services according to the individual’s level of needServes as a key member of the treatment/service team, assuring complex services produce positive outcomes
    • Developsrelationships with the individual and other important people in his/her life asidentifiedand with consent of the individual served
    • Assesses individual strengths and needs through individual and collateral interviews and reviews of social and clinical information provided by other entities
    • Promotes service planning efforts which result in developing,documentingand implementing a comprehensive service plan driven by the individual utilizingall oftheagreed uponstrengths and needs, in close collaboration with the consumer.
    • Investigatesnew resourcesand communicates with directors of prospective resources as a liaison on behalf of the individual being served.
    • Maintains an up-to-date catalog of available community resources.
    • Convenes andfacilitatesinterdisciplinary service planning meetings or other related team meetings to ensure appropriateness and responsiveness of services in relation to individual needs.
    • Ensuresindividuals being served needs are met through theutilizationof naturalsupports, community and generic services and specialized services.
    • Advocates for and with the individual being served to ensure responsiveness from natural, community, generic and specialized services/supports.
    • Ensuresweekly contact withindividualbeing served as well as a minimum of one (1)face-to-face contact in the consumer’s residence toassistindividuals tobuild onstrengths and achieve goals.
  • Quality Improvement
    • Evaluates all services received by the individuals who are served.
  • Supervision
    • Receives Supervision.
    • Receive supervision from and actively communicate with supervisor on an ongoing basis throughout the week by phone, email, written message and when possible, in personregardingprogram matters.
  • Risk Management and Safety
    • Maintainsan accurateandtimelyrecord of Bridges activities.
    • All documentation is completed in SACWIS withfive (5)calendar days of event.
    • All plans and reviews are completed andsignedwithinrequiredtime frame(45 daysforinitialplan,90 daysforinitialreview, every180 daysthereafter).
    • On-going eligibility documents are gathered andsubmittedbyrequiredtime frame(every 180 days).
    • Critical incidents are reported (within 12 hours of incident notification) and documentation issubmittedto state (within48 hoursof incident notification).
    • Monthly budgets are completed andsubmittedby the assigned due date.
  • Professional Development
    • Attends andparticipatesin all required and other professionaltrainings.
    • Seeks out resources and learning opportunities to foster ongoing professional growth, independently.

Qualifications

  • Education/Credentials/Licensure:
    • Requires a bachelor’s degree in human services field.
    • Requires a valid state driver’s license, safe driving record, and current auto insurance.
  • Experience:
    • Requires two (2) years of supervised experience with children and/or families.
  • Clearances:
    • State Police
    • FBI
    • Child Abuse Clearances
    • CPSL Mandated Reporter-Recognizing and Reporting Child Abuse training per state regulation.

Working Conditions

  • Physical Demands:
    • This position requires operating a keypad device 25% of the day and electronic/office equipment 25% of the day. Travel is required approximately 5 times per month and works in weather conditions about 25% of the day. Travel includes transporting students to and from school, school-based appointments, and/or field trips. Fingers, thumbs, hands, wrists, elbows, legs and feet are all routinely used throughout the day. The lower and upper torso is used 75% of the day. Employee must meet minimum requirements for hearing, speech, and vision. Walking is required 50% of the day. Standing occurs 25% of the day. Sitting happens 25% of the day. Reaching above the shoulder as well as bending and twisting are performed 25% of the day. This function routinely lifts 15 pounds, 4 times per day. The maximum lift is 50 pounds, 1 time per day. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.
  • Mental Demands:
    • Continually handles distractions that interfere.
    • Frequently performs multiple tasks, make quick decisions, and concentrate.

    • Non-traditional schedule/hours. Flexibility to meet children/family and program needs.
    • Participates in on-call rotation in a back-up capacity.
