Responsibilities include in-home visits, educating families, connecting patients with resources, and collaborating with facility staff to ensure seamless care transitions.

Qualifications

• Be at least 18 years old.
• At least 1 year in healthcare marketing (preferred).
• Possess a valid driver's license, clean driving record, and reliable automobile.
• Provide proof of current automobile insurance (MVR required).
• Have a clear negative TB test.

Compensation: $19-$21/hr

Status: Full-Time, W2 Employee

Schedule: Monday - Friday, 8 AM - 5 PM

About The Role

We are seeking a compassionate and dynamic Bridge Care Coordinator to join our Bridge Program Team. This position is a critical part of enhancing the care experience by bridging the gap between home health and hospice services. You'll help provide peace of mind to families and create meaningful connections with the seniors you serve.

This unique program ensures a smoother transition for our patients by providing consistent, compassionate support. The Bridge Program redefines post-acute care with an added layer of comfort and continuity.
