Bridge Palliative Care Coordinator

Location: Northern Utah/Southeast Idaho
Company: Ascend Home Health & Hospice
Position Type: Full-Time
Compensation: $20–$25/hour DOE

Position Summary

Ascend Home Health & Hospice is seeking a compassionate, organized, and relationship‑focused Bridge Palliative Care Coordinator to support the growth and coordination of our palliative care program. This role serves as a key connection between patients, families, providers, referral sources, and the interdisciplinary care team to ensure patients receive exceptional supportive care focused on quality of life and symptom management.

The ideal candidate is highly motivated, professional, detail‑oriented, and passionate about serving patients with serious illness while helping guide them through available care options and resources.

Responsibilities
  • Coordinate and support daily operations of the Bridge Palliative Care program
  • Communicate with referral sources, providers, patients, and families regarding services and eligibility
  • Assist with scheduling patient visits and provider coordination
  • Track referrals, admissions, and patient follow‑up needs
  • Maintain accurate documentation and communication within the EMR systemSupport patient and family education regarding palliative care services
  • Collaborate with clinical and administrative leadership to improve workflow and patient experience
  • Build and maintain positive relationships with community partners and referral sources
  • Participate in program development and growth initiatives
  • Assist with care coordination and transitions between care settings when appropriate
Qualifications
  • Experience in healthcare coordination, medical office operations, hospice, home health, or palliative care preferred
  • Strong communication and customer service skills
  • Excellent organizational and multitasking abilities
  • Ability to work independently and collaboratively within a team environment
  • Comfortable working with patients and families facing serious illness
  • EMR experience preferred
  • Knowledge of healthcare terminology and insurance processes is beneficial
