Job summary

Adaptive Home Health in Oak Brook, IL seeks a Clinical Manager to lead clinical execution at the branch with high standards and intensity. You will own day-to-day clinical quality, staff development, and rehospitalization prevention.

You will coach clinicians, oversee escalation decisions, and ensure compliance, while partnering with centralized QA to improve patient outcomes and safety across the branch.

Qualifications

• Active, unrestricted Illinois RN license.
• Minimum one year of leadership experience at a Medicare-certified skilled home health agency.
• Ability to coach, mentor, and train clinical staff across skill levels.
• Strong clinical judgment and structured prioritization/escalation.
• Willingness to be onsite at the branch as an active clinical leader.
• Ability to communicate clearly across clinical and operational teams.

Responsibilities

• Own branch clinical execution with direct guidance to clinicians as needed.
• Monitor on-call events and respond to urgent patient status changes.
• Lead reviews of hospital transfers and root cause analyses for improvements.
• Track quality KPIs and coordinate with centralized QA for documentation accuracy.
