Job summary

Adaptive Home Health is seeking a Clinical Manager to lead clinical execution and staff development at the Oak Brook, IL branch. You will own day-to-day clinical quality, onboarding, and prevention of rehospitalizations.

You will collaborate with operations and leadership to maintain care delivery standards while scaling the branch safely. This role requires active Illinois RN licensure and hands-on leadership onsite.

Qualifications

• Active Illinois RN license.
• Minimum one year of leadership experience at a Medicare-certified skilled home health agency.
• Ability to coach, mentor, and train clinical staff across skill levels.
• Strong clinical judgment and a structured approach to prioritization and escalation.
• A strong sense of personal accountability and ownership of outcomes.
• Ability to communicate clearly across clinical and operational teams.

Responsibilities

• Provide direct clinical guidance to clinicians as needed.
• Respond to clinical calls from patients, caregivers, and providers with appropriate clinical judgment.
• Monitor daily on-call events to identify urgent patient status changes.
• Perform real-time evaluation reports from assessing clinicians to identify patient needs and ensure timely physician orders.
• Complete transfer reviews daily to assess hospitalized patients and plan improvements.
• Review high-risk patients and coordinate proactive interventions to prevent avoidable events.
• Coach clinicians on clinical reasoning, visit planning, and documentation quality.
• Lead new clinician orientation and skills validation before independent deployment.
• Build and lead your branch clinical team as it grows.
• Own branch survey readiness and track quality KPIs and trends.
• Partner with centralized QA on documentation accuracy and OASIS review.
• Conduct field supervisory visits covering infection control and safety.
