Job summary

COPC is seeking an experienced full-time licensed NP/PA to join Grandville Pike Family Physicians in Lancaster, OH. This outpatient role requires strong assessment, preventive care, and chronic disease management with a collaborative team.

Office hours are 8:00–5:00 Monday to Friday, with competitive pay and comprehensive benefits. Training on EPIC is available; CME and PTO are provided.

Qualifications

• Master’s degree in nursing or physician studies from an accredited program.
• Current licensure as a Nurse Practitioner in Ohio.
• National certification as a Family Nurse Practitioner (FNP).
• 1 year outpatient NP experience preferred.
• Strong clinical assessment, diagnostic and treatment skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and with a multidisciplinary team.
• EPIC EHR experience is helpful; onboarding training provided.

Responsibilities

• Conduct thorough assessments, including medical history reviews and physical examinations.
• Provide a full spectrum of primary care services: well visits, vaccinations, disease prevention.
• Develop individualized treatment plans for chronic diseases and monitor progress.
• Educate patients on health maintenance, disease prevention and self-management.
• Collaborate with physicians, APPs, nurses to coordinate patient care.
• Maintain accurate documentation of encounters and follow-up in EPIC.
