Job summary

Virginia Commonwealth University Health Systems is seeking a Clinical Social Worker III to join the Blood and Marrow Transplant and Cellular Therapy program. Qualifications

Master of Social Work Degree from a Council on Social Work Education accredited program.
Licensed Clinical Social Worker in the State of Virginia preferred.
CSW Level 3 requires minimum of three years of clinical social work experience with at least one year in a specialty practice.

Responsibilities

Provide psychosocial assessments and risk assessments.
Coordinate discharge planning and transition planning.
Connect patients and families with community and financial resources.
Educate patients, family members, staff and social work students; advocate for patients and families.
