Job summary

Hackensack Meridian Health is seeking a Transitions of Care Doula to provide continuous physical, emotional, and informational support to birthing people and families before, during, and after childbirth. This hybrid role combines traditional doula support with essential care coordination across New Jersey.

You will advocate for patient choices, collaborate with nurses and providers, and coordinate discharge planning while connecting families to community resources and follow-up care.

Qualifications

• GED or higher required.
• 1+ year as a certified Doula within last 3 years.
• Reliable transportation for travel within service area and 24/7 availability.
• Knowledge of social needs and impact on maternal/infant outcomes.
• Strong organizational, critical thinking, and problem-solving skills.
• Ability to provide culturally competent, patient-centered support with empathy.
• Experience in fast-paced team environments and ability to prioritize tasks.
• Excellent written and verbal communication skills.
• Proficient in Microsoft Office and/or Google Suite.

Responsibilities

• Provide continuous emotional, physical, and informational support to birthing individuals and families throughout the care journey.
• Advocate for patient choices and communicate with nursing and care teams.
• Collaborate with providers, nurses, and staff to ensure seamless, personalized care.
• Offer continuous support during active labor and delivery.
• Support with position changes, hydrotherapy, hydration, and comfort measures.
• Provide reassurance, encouragement, and coping strategies for anxiety.
• Coordinate post-discharge plans and connect families with resources.
• Document care and resources in the patient record.
• Communicate changes to the care plan with the team.
• Support discharge planning and barriers removal as needed.
