Job summary

Township High School District 214 seeks a Prevention Initiative Home Visitor to support pregnant moms and families with children birth to age three in home settings. The role uses Baby TALK to deliver parenting education, child development monitoring, and referrals to services.

Requires a Bachelor’s degree in education or related field, CPR/First Aid, 2+ years experience, and bilingual English/Spanish communication.

Qualifications

• Bachelor's degree in Childhood Education preferred or in the area of social work, human services, family studies, psychology, counseling, or related sciences required.
• Must have or obtain CPR/AED and First Aid certificates.
• Must have 2 years of experience in a related field required.
• Must submit to background check through District 214.
• Must complete new employee GCN training and update annually as required.
• Must complete the required BabyTalk training and others immediately following employment.
• Must be able to work in a socially-and psychologically pressured environment.
• Preference given to those with home visiting and/or group facilitation experience.
• Preference given to those with Baby TALK training or similar model.
• Must have proficiency in English and Spanish.
• Must have excellent oral and written communication skills.

Responsibilities

• Implements the Baby Talk Curriculum within the home environment and socialization group.
• Maintains a full caseload as assigned, typically 12 families.
• Facilitates two home visits per month to assigned caseload, and completes 100% of home visits per year.
• Coordinates activities and supports the family during the transition plan.
• Documents and maintains individual records on children including home visits, screenings, assessments, referrals, and progress reports.
