Job summary

The Township High School District 214 Community Education Foundation seeks a Prevention Initiative Home Visitor to support prenatal and infancy home-based services at Forest View Educational Center. Bachelor’s degree in Childhood Education preferred or related fields required.
Must have or obtain CPR/AED and First Aid certificates.
Must have 2 years of experience in a related field.
Must submit to background check through District 214.
Must complete BabyTalk training and other required trainings.
Must be able to work in a socially and psychologically pressured environment.
Must have proficiency in English and Spanish.
Must be able to use technology-related programs (Google Suite, Microsoft Suite, NewOrg).
Must be able to lift up to 20 pounds and perform physical tasks.

Responsibilities

Provides home visiting services to pregnant moms and families with children birth to age three.
Implements the Baby Talk Curriculum in the home and group settings.
Maintain full caseload and conduct two home visits per month.
Identify family needs and provide parenting education and coaching.
Coordinate referrals to community services and collaborate with other staff.
Document and maintain records, including progress notes and assessments.
Attend trainings, meetings, and supervision as scheduled.
