Job summary

The Community Youth Center of San Francisco (CYC) is seeking a Community Social Worker to provide culturally responsive, trauma‑informed case management for youth and families across San Francisco. You will identify needs, develop individualized plans, and link clients to housing, healthcare, education, and public benefits.

This full‑time, exempt role reports to the Program Director and requires experience with diverse communities and bilingual Cantonese or Mandarin and English.

Qualifications

• Master’s degree in Social Work, Counseling, Psychology, Marriage and Family Therapy, or a related field.
• Minimum of two years of case management, behavioral health, or related services.
• Experience with San Francisco’s diverse communities and populations.
• Bilingual proficiency in Cantonese or Mandarin and English required.
• Valid California driver’s license and clean driving record.

Responsibilities

• Conduct client intakes, needs assessments, and ongoing service planning.
• Provide trauma‑informed, strengths‑based, client‑centered case management.
• Develop goals and individualized service plans and monitor progress.
• Connect clients with housing, healthcare, behavioral health, education, employment, and public benefits.
• Provide referrals, care coordination, and follow-up for linkage to services.
• Maintain accurate, confidential case records and documentation.
