COMHAR is seeking Targeted Case Managers to join our team in Philadelphia, PA. You will work directly with adults, including young adults (18-25) and Spanish-speaking community members, to assess needs, develop service plans, and coordinate community-based supports.

You will meet clients in homes or community settings, providing hands-on guidance and advocacy to help them navigate services and achieve goals.

Qualifikationen

• High school diploma or GED with 12 college credits in behavioral health/human services.
• Associate's or Bachelor's degree in Social Work, Psychology, or other Human Service field preferred.
• 2–4 years of CMI experience.
• Bilingual proficiency in English and Spanish preferred but not required.
• Current Driver's License; eligible to operate program vehicles.

Aufgaben

• Assess clients' needs and elaborate service plans to promote independence.
• Maintain clients' records by reviewing case notes and service plans.
• Submit documentation for all services provided.
• Participate in daily meetings and supervision.
• Support individuals in crisis management and de-escalation.
• Establish treatment programs and coordinate services, including transportation.
• Monitor attendance, advocate for services, and obtain resources.
• Meet productivity requirements and provide community-based services.
