Comhar Inc. in Philadelphia, PA is seeking a Bilingual Targeted Case Manager to empower adults, including young adults and the Hispanic community, through individualized service plans and hands-on guidance.

You will conduct home, hospital, and community visits, coordinating supports to help clients navigate services and meet goals. You will work with a bicultural team to assess needs, develop plans, and advocate for resources, with a schedule Monday–Friday 8:30AM–5:00PM and bilingual

Qualifications

• High school diploma or GED with 12 college credits in behavioral health/human services required.
• 2–4 years of CMI experience.
• Bilingual proficiency in English and Spanish; reading and writing skills preferred.

Responsibilities

• Assess needs and develop service plans; monitor mental and physical health; link to community resources.
• Review case notes and service plans; maintain client records.
• Submit documentation for services provided; ensure timely processing.
• Attend daily meetings and supervision; support individuals in crisis management and de‑escalation.
• Establish treatment programs by scheduling and coordinating services; arrange transportation.
• Monitor cases by verifying attendance and evaluating treatments; advocate for needed services.
• Maintain productivity targets and meet clients where they live to provide community-based services.

Job Summary

COMHAR is seeking Bilingual Targeted Case Managers to join our team. In this role, you will work directly with adults, including young adults (18–25) and members of the Hispanic community through our bicultural/bilingual team, to assess needs, develop service plans, and coordinate community-based supports. You will meet clients in their homes, hospitals, or other community settings, providing hands‑on guidance and advocacy to help them navigate services and achieve their goals.

Key Responsibilities

• Accomplishes clients' goals and objectives by assessing needs and elaborating service plan; monitoring the client's mental and physical health, and linkage to community resources in order for the client to gain the needed tools to navigate the system and promote independence.
• Maintains clients' records by reviewing case notes; and service plan reviews.
• Submitting documentation for all services provided.
• Participating in daily morning meetings and individual supervision.
• Supporting individuals in crisis management and verbal de‑escalation.
• Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation.
• Monitors cases by verifying clients' attendance to providers; observing and evaluating treatments and responses; advocating for needed services in the community; obtaining additional resources
• Maintaining productivity requirements of the program.
• Meeting individual where they are and providing services in the communities they live in.

Requirements

Targeted Case Manager Job Requirements:

• High school diploma or GED with 12 college credits in behavioral health/human services required.
• Associate's degree or Bachelor's degree (must be in Social Work, Psychology, or other Human Service field) preferred.
• 2–4 years of CMI experience.
• Bilingual proficiency in English and Spanish, with strong reading and writing skills is preferred but not required.
• Current Driver's License; satisfactory driving record; eligible to operate program vehicles.
