Job summary

Horizon Services, Inc. in San Leandro, CA is seeking a SUD Counselor I to support diverse clients in residential and detox programs.

The role includes admissions, assessments, treatment planning, documentation, and discharge coordination, with a focus on accessibility and quality care. The position offers a full-time schedule with a swing shift (2pm–10:30pm) and opportunities to work with multilingual clients.

Qualifications

• Certification or registration with California state certifying body (CAADE/CCAPP/CADTP).
• Preferred bilingual abilities: English plus Spanish/Chinese/Vietnamese/Tagalog.
• Excellent computer skills and data input into EHR systems.
• Strong oral and written communication skills.
• Knowledge of alcohol and other drug addiction and detox concepts.

Responsibilities

• Perform client admissions, transfers, assessments, referrals, and discharges daily.
• Coordinate with Clinical Supervisor to ensure services align with policy.
• Maintain positive welcoming atmosphere for detox clients.
• Input data into EHR and maintain accurate records.
• Monitor patient progress and support discharge planning.
