Who We Are: CAMBA is a community organization comprising staff, volunteers, clients, donors, neighbors, and partners dedicated to building an inclusive New York City. Our mission is to ensure all children and adults have access to resources and support to thrive. We offer over 180 integrated programs across Education & Youth Development, Family Support, Job Training & Employment Support Services, Health, Housing, and Legal Services, reaching more than 73,000 individuals and families, including nearly 13,000 youth. CAMBA serves a diverse population, including new mothers in Brownsville and job seekers in the Rockaways, with over half of our clients being immigrants and refugees. More than 85% of our families live in poverty, reflecting the challenges faced by nearly 1.7 million New Yorkers today.

HOPWA scattered-site housing provides 50 supportive housing units for PLWHA, primarily throughout Brooklyn. The case management staff focus on removing barriers to maintain housing by ensuring clients’ basic needs are met and connecting them to services such as mental health and substance abuse treatment. The operations team works on maintaining habitability standards, addressing repairs, identifying new units, negotiating leases, and preparing units for new tenants. Clients contribute 30% of their income towards rent.

Position: Bilingual Case Manager

Reports To: Program Manager

Location: 19 Winthrop Street, Brooklyn, NY 11225

What the Bilingual Case Manager Does:
  • Provide all contract-funded activities, including conducting and completing Intakes, Assessments, Service Plans, Reassessments, Service Plan Updates, Accompaniment, Referrals, Advocacy, Housing Inspections, Case Conferences, etc. Ensure all client services are documented and entered into Client Track. Assist clients in maintaining primary medical care and adhering to treatment.
  • Connect clients to necessary services such as medical, mental health, substance use treatment, legal aid, and entitlements.
  • Monitor clients' progress towards their goals through scheduled contact and visits; document progress notes.
  • Act as a liaison and advocate with outside organizations regarding education, healthcare, housing, legal issues, etc.
  • Escort clients to appointments (educational, medical, social services).
  • Assist clients with benefits and entitlements applications; monitor outcomes.
  • Collect documentation and maintain client files.
  • Conduct at least one home visit per month.
  • Report repair issues to the Housing Specialist.
  • Monitor clients’ living conditions.
  • Meet monthly projections and provide information for program reports.
  • Maintain client charts in an audit-ready state.

Minimum Education/Experience Required:

  • Bachelor's or Associate degree with one year of relevant experience, or
  • Two years of equivalent experience with sufficient education and technical skills to understand work orders, perform basic maintenance tasks, and document work.

Other Requirements:

  • Relevant experience, preferably in a mid-sized organization working with individuals experiencing homelessness, mental illness, and/or substance abuse.
  • Strong written and verbal communication skills.
  • Proficiency in Microsoft Office Suite.

Compensation: $51,500 annually. Salary offers depend on experience, education, program size, internal equity, skills, and other factors.

Status: Full-time (35 hours/week)
