Alignment Health is seeking a bilingual Spanish care coordinator to join the remote Care Anywhere team, who has front office medical assistant experience supporting providers. The Care Coordinator is responsible for scheduling, organizing and managing all aspects of a patient’s healthcare maintenance and treatment. Helps patients complete paperwork, communicates with a healthcare team about a patient’s treatment plan and educates patients about resources and options for managing their health.

Schedule: Monday - Friday, 8:00 AM - 5:00 PM Pacific Time

GENERAL DUTIES/RESPONSIBILITIES (MAY INCLUDE BUT ARE NOT LIMITED TO):

  • Handle 60–80 combined inbound and outbound calls per day to support patient care coordination, including appointment scheduling, follow-ups, and provider communication.
  • Reviews Daily Census for new Admissions
  • Obtains medical records from Provider offices, hospitals and SNF’s and attaches medical records to authorizations.
  • Enters referral requests/authorizations in system
  • Creates cases and tasks in Case Management module for Hospital and SNF discharges.
  • Schedules post discharge appointments
  • Assist NP team with visit preparation needs
  • Appointment reminders to members
  • Assign members to NP in EHR
  • Enter all medications and allergies in EHR
  • Provide needed documentation to NP for visits each day
  • Directs inbound calls from members/family related to medication refills
  • Notifies NP/nurse If members appear to be non-compliant or there appears to be a change in condition
  • Assists with outreach activities to members in all levels of Case Management Programs.
  • Assists with maintaining and updating member’s records
  • Assists with mailing or faxing correspondence to PCP’s, Specialists, related to, as needed.
  • Attends Case Management presentations and participates, as appropriate.
  • Recognizes work-related problems and contributes to solutions.
  • Works with outside vendors to provide appropriate care needs for members
  • Other duties as assigned by CM Manager or Director of Case Management.

Job Requirements:

Experience:

• Required: Minimum 1 experience entering referrals and prior authorizations in a healthcare setting.

• Preferred: 2+ years healthcare experience.

Education:

• Required: High School Diploma or GED.

Training:

• Preferred: Medical Terminology

Specialized Skills:

• Required:

  • Ability to communicate positively, professionally and effectively with others; provide leadership, teach and collaborate with others.
  • Knowledge of ICD9 and CPT codes
  • Knowledge of Managed Care Plans
  • Basic Computer Skills, 25 WPM (Microsoft Outlook, Excel, Word)
  • Effective written and oral communication skills; ability to establish and maintain a constructive relationship with diverse members, management, employees and vendors;
  • Knowledge working in Access Express/Portal, Epic, essette (not mandatory)
  • Language Skills: Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals. Ability to write routine reports and correspondence. Communicates effectively using good customer relations skills.
  • Mathematical Skills: Ability to add and subtract two-digit numbers and to multiply and divide with 10’s and 100’s. Ability to perform these operations using units of American money and weight measurement, volume, and distance.
  • Reasoning Skills: Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions. Ability to deal with problems involving a few concrete variables in standardized situations.
  • Problem-Solving Skills: Effective problem solving, organizational and time management skills and ability to work in a fast-paced environment.

• Preferred: Bilingual (English/Spanish) preferred

Pay Range: $41,472.00 - $62,208.00
