Families First Pediatrics in West Jordan, UT seeks a Bilingual Social Worker to join our care team on a part‑time basis. You will identify patients in need of medical coverage, behavioral health services, food, housing, and other supports, providing direct outreach in Spanish.

This entry-level role focuses on case management, screening, advocacy, and supportive contacts—no direct psychotherapy. Travel between southern clinics is required with mileage reimbursement.

Qualifications

• Bachelor of Social Work from a CSWE-accredited program.
• Utah Social Service Worker (SSW) license or eligibility with licensure within 90 days of hire.
• Full professional fluency in Spanish and English, spoken and written.
• Valid driver's license and transportation for travel between southern clinics.
• Discretion with confidential patient information and HIPAA compliance.

Responsibilities

• Case management and navigation for referred patients and families.
• Deliver services in Spanish and interpret between Spanish-speaking families and providers.
• Document contacts, referrals, and applications in the medical record promptly and accurately.
• Coordinate with providers and clinic teams, respond to referrals within defined timelines.
• Maintain awareness of confidentiality, safety reporting obligations, and license boundaries.
