Job summary

Klamath County is seeking a bilingual Spanish-speaking Services Coordinator to provide targeted case management for individuals with Intellectual/Developmental Disabilities. The role focuses on developing ISP/Annual Plans, coordinating services, and maintaining confidentiality while navigating Oregon's public service system.

Ideal candidates will have a degree in behavioral or social sciences (or equivalent experience) and be fluent in Spanish. A valid Oregon driver's license is required.

Qualifications

• Bachelor’s degree in behavioral science, social science, or a closely related field, or equivalent experience in human services.
• Bachelor’s degree in any field plus one year of human services related experience.
• Associate’s degree in a behavioral or social science field with two years of related human services experience.

Responsibilities

• Develop and periodically revise an ISP or Annual Plan based on assessments and goals.
• Assist individuals and families in developing a support system from a Needs Assessment.
• Utilize choice advising to help individuals select services, providers, and settings.
• Develop plans and monitor for effectiveness and Medicaid standards.
• Deliver services using person-centered practices and encourage self-determination.
• Authorize and deliver developmental disabilities services chosen by the individual or their representative.
• Coordinate with state, public, and private agencies regarding services.
• Maintain knowledge of laws, rules, contracts, and regulations.
• Independently maintain a caseload of individuals in various service elements.
• Use calendars and software to manage workload tasks.
• Document interactions with weekly progress notes.
• Attend supervision and participate in case consultation and training.
• Participate in transition planning (IEP meetings).
• Deliver case management in compliance with OAR 411-415.
• Report existing and insufficient service delivery resources.
• Complete at least 20 hours of I/DD training per year.
• Develop relationships with community partners and stakeholders.
• Maintain confidentiality and act as a mandatory abuse reporter.
• Promote KCDDS mission and values with professionalism and integrity.

This position provides targeted case management services to individuals with Intellectual/Developmental Disabilities (I/DD). Ideal Candidate

Our Bilingual Services Coordinator is fluent in Spanish and has knowledge of the public service system for developmental disabilities services in Oregon. You should have a bachelor's degree in behavioral science, social science, or a related field, or equivalent experience in human services. You are detail-oriented, organized, and capable of managing multiple tasks. You excel in communication and are able to effectively work with individuals with I/DD, their families, and various stakeholders. You have a proactive approach to problem-solving and a commitment to maintaining confidentiality and professional boundaries.

Key Responsibilities

Qualifications

• Education (must have one of the following):
  • A bachelor's degree in behavioral science, social science, or a closely related field; or
  • A bachelor’s degree in any field and one year of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing; or
  • An associate’s degree in a behavioral science, social science, or a closely related field and two years of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing
• Certifications: Valid Oregon Driver’s License
• Skills and Abilities:
  • Fluency in Spanish (reading, writing, and speaking)
  • Knowledge of the public service system for Developmental Disability Services in Oregon
  • Familiarity with Klamath County community agencies and resources
  • Understanding of mandatory reporting requirements and confidentiality rules
  • Proficiency in using Microsoft Office, record-keeping programs, and other relevant software
  • Ability to manage complex projects, collaborate effectively in a team environment, and maintain professional boundaries
  • Strong communication skills in English and Spanish, with effective presentation and public speaking abilities
  • Capacity to handle highly sensitive and confidential information appropriately
