Qualifications

• Three years of experience as a Registered Nurse required, preferably in Emergency Room, Urgent Care, Critical Care, or Clinic setting.

Responsibilities

• Navigate, coordinate, and assist patients requiring clinic and community resources.
• Conduct population health management to meet patient care needs.
• Coordinate resources and navigate within clinics according to regulatory standards.
