Martin Luther King, Jr. Community Hospital seeks an RN Care Manager for our Emergency Room to coordinate patient care from arrival through safe discharge.

You will perform thorough assessments, develop and update care plans, and collaborate closely with physicians, social workers, and the interdisciplinary team to ensure timely, appropriate services and smooth transitions of care. You will educate patients and families, advocate for health equity, and connect individuals with community resources

Responsibilities

• Coordinate patient care from ER arrival through safe discharge.
• Perform thorough assessments and develop/update care plans with the care team.
• Collaborate with physicians, social workers, and interdisciplinary team to ensure timely, appropriate services and smooth transitions of care.
• Educate patients and families, advocate for health equity, and connect individuals with community resources.
• Monitor utilization, quality, and safety indicators and document in the EHR.
• Participate in huddles, case conferences, and performance improvement initiatives focused on throughput and outcomes.
