Qualifications

• Requires RN licensure.

Responsibilities

• Coordinate patient care from ER arrival through safe discharge.
• Perform thorough assessments and update care plans.
• Collaborate with physicians, social workers, and the interdisciplinary team to ensure timely services.
• Educate patients and families; advocate for health equity.
• Connect patients with community resources; support recovery and long-term wellness.
• Monitor utilization, quality, and safety indicators; document in EHR.
• Participate in huddles, case conferences, and performance improvement initiatives focusing on throughput and outcomes.
