Job summary

Diamond Experience Co. in Houston, TX, seeks a Qualified Mental Health Professional/Case Manager (QMHP) to provide targeted case management and rehabilitative services to youth with serious emotional disturbances and their families.

You will work with the clinical team to stabilize children and youth, using a strengths-based approach to build skills for autonomous family functioning and improved well-being.

Qualifications

• Must have at least a Bachelor's in Social Work, Mental Health Counseling, Psychology, Sociology, or related field.
• CANS/ANSA Certification required.
• Bilingual- English and Spanish required.
• At least two years prior experience in behavioral health.
• Must have reliable transportation, valid driver’s license, auto insurance, and clean driving record.
• Must have access to computer and internet.

Responsibilities

• Provide Targeted Case Management and rehabilitative services to youth with serious emotional disturbances and their families.
• Develop goal-directed interventions to stabilize children and youth in transition.
• Maintain all client documentation in the EHR and write progress notes per regulatory requirements.
• Collaborate with clinical team to develop service/treatment plans focused on client needs and strengths.
• Provide skill-building training in home or community settings using evidence-based practices.
• Adhere to Medicaid/Insurance guidelines and participate in staff meetings and training.
