Job summary

Hand In Hand Development, Inc. seeks NYS Licensed Physical Therapists to provide in-home services across NYC. The role uses a team-centered approach to support children with developmental delays, working closely with families to implement programs and monitor progress.

Travel to client homes is required; you will document outcomes and coordinate with our multidisciplinary team to ensure effective care and family empowerment.

$65 for 30 min session, $85 for 60 min session without DOH Letter as a W2

$70 for 30 min session, $90 for 60 min session with DOH Letter as a 1099

Essential Duties and Responsibilities:

We are seeking NYS Licensed Physical Therapists to provide therapeutic services in home settings. Our In-Home Services are provided throughout the 5 boroughs of NYC. We utilize a team centered approach fostering collaboration across all disciplines.

• Work 1:1 with children diagnosed with developmental delays
• Collaborate with families to create programs and work towards goals listed in the child’s IFSP
• Monitor & assess program effectiveness
• Document & communicate treatment results & individuals progress to relevant parties
• Work effectively with our team of multi-disciplinary service providers

Education and/or Work Experience Requirements:

• Master's in Physical Therapy (Required)
• NYS Physical Therapist License (Required)
• CPR Certification (Preferred)
• Previous experience with children who have disabilities and families
• Ability to build rapport with children & families
• Passion to help & motivate others

Physical Requirements:

• Ability to travel to numerous locations to meet with clients, attend meetings and visit healthcare facilities.
• Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
• Must be able to lift and carry up to 35 lbs.
• Must be able to talk, listen and speak clearly.
