Qualifications

Responsibilities

• Provide direct clinical services including psychosocial assessments and therapy.
• Develop treatment plans and coordinate care with families and other providers.
• Lead or participate in interdisciplinary team meetings.
• Provide psycho-educational training and discharge planning.
• Document patient data and plans in accordance with KP guidelines.
• Offer consultation for complex cases and care coordination.
