Job summary

Central-Jersey-Family-Health-Consortiu is seeking a Program Support Specialist for the Family Connects home visiting program. The role links parents/caregivers of newborns to enrollment, including families affected by infant loss, and records data on the eligible birth population.

Coordinate nurse schedules, recruit families, and collaborate with hospitals; conduct post-visit follow-ups, text confirmations, and referrals while maintaining strong English/Spanish communication and meticulous data

Qualifications

• Education: Associates degree required; Bachelor’s degree preferred in education or health related field required.
• Verbal and written fluency in English and Spanish; strong data entry skills and attention to detail.
• Experience in nonprofit, maternal child health or related fields preferred.
• Experience in healthcare or social services and patient/client settings preferred.
• Able to work well in a team and independently to meet deadlines.
• Sensitivity to diverse populations and underserved communities; strong interpersonal skills.

Responsibilities

• Coordinate scheduling of all nurse assignments and manage the client database.
• Assist with client recruitment and recruit/schedule families for nurse home visits.
• Engage with hospital birth center staff; attend meetings and provide in-services as needed.
• Schedule nursing visits, via phone or in-hospital visits when appropriate.
• Conduct post-visit connection calls; track services and provide re-referrals as needed.
• Manage appointment confirmations via text messaging.
• Serve as a primary contact to answer questions and reschedule visits; Follow up promptly.
• Participate in regular staff meetings and trainings; perform other duties as assigned.
