Job summary

Nationwide Healthcare Services seeks a Personal Care Assistant to provide comprehensive in-home and center-based care for frail elders. The role involves assisting with ADLs, IADLs, mobility, and meals across care settings, working with therapists and the Interdisciplinary Team to implement the plan of care.

Requirements include Delaware-approved PCA training, CNA or Home Health Aide credentials, and experience with elderly populations.

Qualifications

• Completion of a Delaware-approved Personal Care Aide training program or approved equivalency test.
• Experience as a homemaker, Personal Care Aide, Home Health Aide, or Nursing Assistant preferred.
• Interdisciplinary teamwork ability; cooperation with diverse staff and participants.
• Age of patients served includes adult and geriatric populations.

Responsibilities

• Provide physical assistance and attendant care across care settings (Center, home, transportation).
• Assist with activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
• Assist with mobility, transfers, and ROM under supervision of therapists or Home Care Coordinator.
• Assist with meals, medication management, and light homemaking tasks.
• Keep accurate records and communicate changes in health status to the team.
