South Plains Rural Health Services seeks a Patient Care Coordinator to navigate referrals, coordinate screenings, and keep chronic care on track. You will collaborate with providers and nursing staff, help patients schedule follow-ups, and maintain secure patient records under HIPAA.

The role emphasizes care coordination, data entry in the EMR, and population health support within a community health center setting in Texas.

Competenze

• Medical Assistant or CNA preferred.
• At least one year of experience in a clinical setting; community health center experience preferred.
• Working knowledge of EMR systems, care coordination, and chronic disease support.
• Strong organization, communication, and follow-through skills.
• Ability to work collaboratively and protect patient confidentiality.
• Bilingual in English and Spanish preferred.

Mansioni

• Coordinate outgoing referrals and imaging orders with providers and Referral Department.
• Follow up with patients about scheduling, steps, and access to care.
• Track records and results, update EMR, ensure referrals are completed.
• Partner with providers and nursing staff to support care coordination.
• Maintain patient registries for diabetes and hypertension; contact overdue patients.
• Support population health, UDS reporting, PCMH activities, and quality projects.
• Assist with PDSA activities to test and improve care processes.
• Maintain accurate, confidential patient records per HIPAA and SPRHS policies.
