EPIC Health, a multi-specialty practice in Chicago, seeks an Advanced Practice Provider (PA/APRN/FNP) to join our on-site team in the Northwood Park-Portage Park area. This role emphasizes patient-centered care in a fast-paced environment.

Qualifications include IL PA or APRN licensure, a Master’s degree, 2–5 years in primary care/pediatrics, and bilingual English and Polish proficiency. Base pay ranges $100,000–$140,000 with up to $9,000 quarterly bonuses and student repayment options.

Qualifications

• IL PA or APRN licensure required.
• Master’s Degree in a related field.
• Bilingual in English & Polish required.
• 2–5 years in Primary Care and Pediatrics preferred.

Responsibilities

• Own patient care: examine patients, gather histories, evaluate conditions and diagnose.
• Collaborate with physicians: prescribe therapies/medications with physician approval and coordinate care.
• Lead diagnostic efforts: administer and interpret tests (x-rays, EKGs, labs).
• Provide holistic care: counsel on health regimens, development, family planning, prevention.
• Perform procedures: injections, suturing, wound care, immunizations.
