Job summary

Green Key Resources is seeking a Care Manager who speaks Mandarin or Cantonese to serve as the primary point of contact between members, families, providers, and the health plan in New York.

You will build relationships with members through ongoing communication, review person-centered service plans, coordinate benefits and services, and collaborate with families, providers, and vendor agencies to ensure continuity of care.

Qualifications

• Mandarin or Cantonese fluency required.
• Experience in care management or healthcare preferred.
• Strong communication and relationship-building skills.

Responsibilities

• Build relationships with members through monthly communication and ongoing follow-up.
• Review person-centered service plans with members and their families.
• Coordinate covered and non-covered benefits and services.
• Collaborate with families, providers, and vendor agencies to ensure continuity of care.
• Identify member needs and address concerns promptly.
• Maintain timely and thorough documentation in member charts.
• Participate in weekly interdisciplinary team meetings.
