Job summary

Suvida Healthcare is seeking a Nurse Care Manager to join its multidisciplinary team, delivering high‑quality care for high‑risk Hispanic seniors in a culturally responsive program. You will coordinate chronic care, transitions of care, and social determinants of health across neighborhood centers, working with clinicians, payers, and families.

The role requires an RN license valid in Arizona, 1+ year of relevant experience, and a willingness to collaborate across disciplines in a hybrid work

Qualifications

• Minimum 1 year of experience as a Registered Nurse.
• Experience in utilization management, case management, chronic care management, discharge planning, transitions of care management, or related field.
• Bilingual English and Spanish required.
• Willingness to work a hybrid schedule: 2 days in clinic, 3 days remote.

Responsibilities

• Oversees chronic care and transitions of care for high-risk patients within care teams.
• Collaborates with the multidisciplinary team to develop and implement care plans.
• Performs triage calls and coordinates transitions from acute to home or other settings.
• Ensures documentation and communication across the care continuum for assigned patients.
• Promotes quality improvement, data-driven decision making, and patient satisfaction.
