Job summary

Piedmont Health Services, Inc. is seeking an Outreach & Enrollment Coordinator for Burlington, NC, working to promote NC Medicaid access and enrollment for uninsured individuals. The role requires flexible hours including some evenings and weekends, and involves outreach, case management, and health education.

The position reports to the Outreach & Enrollment Manager and is based at Burlington CHC. Schedule is typically 8:00am–5:00pm with potential Saturdays; travel as needed.

Qualifications

• Bilingual required.
• Two years of experience in community service, health or social service sector; NC Medicaid Ambassador and/or Navigator program experience preferred.
• EMR EPIC experience strongly preferred.
• Current NC Driver’s License.
• Immunizations up-to-date prior to starting employment.

Responsibilities

• Serve as the point of contact for individuals and/or families contacted during off-site outreach activities regarding public health insurance enrollment.
• Routinely visit with individuals and their families to learn details about their health and provide information on available services.
• Provide comprehensive and culturally sensitive information about public health insurance programs.
• Assist eligible individuals and families in identifying documents needed to complete applications and answer questions pertaining to the process.
• Refer individuals to Certified Application Assistants for application assistance based on eligibility.
• Ensure confidentiality of all applications and records.
• Conduct follow-up on enrollment progress.
• Coordinate outreach enrollment with community partners and organizations.
• Participate in outreach events and other duties.
• Document encounters with individuals and families.
