Job summary

The Children's Home of Poughkeepsie is seeking a bilingual Clinician to join the UC LTFC program. You will perform mental health assessments, provide individual therapy, and respond to crises for placed UC, coordinating with Case Managers for reunification plans and reporting. Spanish proficiency is required.

You will document in UC Portal, ensure policy compliance, and participate in conferences to optimize care and discharge planning for youth in our care.

Qualifications

• MSW and 2 years of postgraduate direct service delivery experience; licensed or license eligible.
• Proficient in the Spanish language.

Responsibilities

• Conduct mental health assessments for UC in care and provide ongoing individual therapy.
• Deliver crisis intervention and trauma-informed therapy services.
• Collaborate with Case Managers for plan development and reporting to ORR.
• Maintain UC Portal documentation, progress notes, and incident summaries.
• Participate in conferences, staffing meetings, and post-release care planning.
