Urban Health Plan in the Bronx is seeking an LMSW Social Worker II/III for a full-time role. The position supports patients with counseling, crisis intervention, psychoeducation, and advocacy within our Integrated Care team.

It offers a hybrid schedule: four days onsite and one remote day, with potential for clinical hours and growth opportunities. The starting compensation ranges from $67,600 to $75,000 per year.

Qualifications

• Master's degree in social work is required.
• NYS LMSW Certification is required.
• Bilingual in English and Arabic or Spanish is required.
• Experience with crisis intervention is preferred.
• Excellent communication skills are essential.

Responsabilités

• Conduct psychosocial assessments and provide supportive counseling.
• Perform crisis intervention and complex case management.
• Collaborate with interdisciplinary team to address psychosocial barriers.
• Refer patients to behavioral health services and community resources.
• Document encounters in a timely manner while maintaining confidentiality.
