Responsibilities

• Identify medical-related social needs and provide appropriate services, maintaining records.
• Consult with the Director of Social Services and other departments on interdisciplinary issues and records.
• Work with residents and families facing personal or environmental difficulties related to health.
• Promote residents' physical and mental health and prevent problems.
• Maintain written records of social service consultations and services provided.
• Evaluate residents’ social needs and update care plans in the medical record; re-evaluate periodically.
