Job summary

Wheeler Clinic seeks a Bilingual MDFT Mental Health Counselor to provide intensive in-home clinical care coordination for adolescents facing substance use and delinquent behaviors, along with their families.

Role requires fluency in English and Spanish, a BA/BS degree, and at least 3 years in human services. Valid driver's license is required; full-time schedule in Waterbury, CT with a strong emphasis on documentation and coordination of care.

Qualifications

• BA/BS degree required with 3+ years in human services
• Ability to speak fluently in English and Spanish
• Valid driver's license required

Responsibilities

• Provide in-home clinical care coordination for adolescents with substance abuse and delinquent behaviors
• Deliver group and individual counseling with supervision
• Develop case management plans and maintain documentation for Medicaid and licensing requirements
• Coordinate referrals and linkages with community providers
• Assist with crisis intervention and safety planning as needed
• Provide translation support to assist clinical care when appropriate
