Job Summary
The Medical Case Management Services of the Partnership Comprehensive Care Practice (PCCP) Outpatient Clinic serves an essential role as part of a multi-disciplinary team that provides patient care and education for adult patients living with HIV. Under the direction of the Medical Case Management Program Manager, this position will provide a range of trauma informed and patient-centered activities focused on improving health care outcomes. Duties include, but are not limited to: Initial and ongoing assessment of service needs, timely and coordinated access to medically appropriate levels of healthcare, collaboration with care providers and medical support staff to support antiretroviral therapy adherence, accompanying a client to a medical appointment with client’s consent, client-specific advocacy and/or review of utilization of service, and positively promotes the Mission, Vision and Values of the Partnership Comprehensive Care Practice and Drexel Medicine.

This position is classified as Exempt, grade K. Compensation for this grade ranges from $54,630 to $81,940 per year. Please note that the offered rate for this position typically aligns with the minimum to midrange of this grade, but it can vary based on the successful candidate’s qualifications and experience, department budget, and an internal equity review.
