Qualifications

• Bachelor's degree or higher in Health Education, Public Health, Nursing, Education, or Nutrition.
• At least 5 years of health education experience.
• Experience in patient counseling and group education.
• Current CPR/First Aid Certification or within 90 days of hire; masking when required by policy.
• TB screening and health requirements as part of pre-hire process.

Responsibilities

• Assess health education needs and provide education on chronic diseases and preventive care.
• Design, promote, present and evaluate community health sessions.
• Develop collaborative relationships with local health, educational and social service providers.
• Attend staff meetings and in-service trainings; submit accurate weekly/monthly reports.
• Review and update health education materials and curricula; create pamphlets as needed.
• Chart notes in the Electronic Health Records (EHR).
• Participate in Literacy Review process for health education materials.
