Job summary

Alignment Health is seeking a compassionate Bilingual Health Coach & Educator to provide in-home support, wellness coaching, and health education to members throughout the service area. The role focuses on building trusted relationships with members, caregivers, and families to promote healthier habits and improved well‑being.

You will work in a field-based capacity with daily local travel, coordinating with clinicians to develop member action plans and connect clients with community resources.

Qualifications

• Bilingual Spanish, must be able to write, read, speak.
• Minimum 1+ year of experience as a wellness health coach or educator.
• Current, valid, unrestricted California Driver's License.
• Reliable personal transportation and ability to travel throughout the assigned territory.
• Ability to maintain automobile insurance in accordance with company requirements.
• Ability to work independently and remotely.
• Strong communication, organizational, time management, and relationship‑building skills.
• Proficiency with Microsoft Office (Excel, Word, and PowerPoint).
• Knowledge of health, wellness, nutrition, exercise, and behavior change principles.
• Ability to maintain HIPAA compliance and confidentiality.

Responsibilities

• Conduct approximately 6-8 in‑home visits daily throughout your assigned service area.
• Provide wellness coaching and health education to support behavior change and healthier lifestyles.
• Partner with Advanced Practice Clinicians and Social Workers to develop member action plans.
• Connect members with community resources and social services.
• Support member engagement, retention, and satisfaction.
• Coordinate care with internal clinical teams and external partners.
• Deliver health education materials to members, caregivers, and families.
• Applicants must be comfortable conducting daily in‑home visits and traveling throughout Fresno, CA.

What You'll Do

What to Expect

• Primarily field-based role with daily local travel.
• Monday-Friday, 8:00 AM-5:00 PM PST.
• Visits are scheduled geographically to maximize efficiency.
• Mileage reimbursement provided from door‑to‑door travel.

Pay Range: $48,373.00 - $72,559.00 Pay range may be based on a number of factors including market location, education, responsibilities, experience, etc.
