• Conduct visits with children, parents, and/or caregivers to assess safety, permanency, and well-being.
• Develop and implement plans meeting the needs of children and family.
• Facilitate placement and delivery of services with families.
• Perform case management in compliance with laws, codes, procedures, and policies within required timeframes.
• Attend court hearings and file documents within required timeframes.

Foster Care Case Manager

The Foster Care Case Manager position carries for DCFS. The ability to engage with diverse families, good oral and written communication skills, valid driver's license, be able to pass all background checks, professional and working relationships with service providers, court personnel, and DCFS staff.

Must be able to multi-task and work well under pressure at times. Have knowledge of computer skills and the ability to be on call at times.

Must pass DCFS required tests to become a Licensed Child Welfare Specialist within 60 days of hire and become Solution-Based Casework certified within 9 months of hire.

Education

• Bachelor's Degree in Human Services Field or Relatedrequired

Certification

• CWEL, CANS, CERAP, placement specialtypreferred
• Must have a good driving record

Other

• Must be bilingual (Spanish/English)

Job Duties Include

• Serve as a conduit of information between all parties to the case and identified stakeholders, including medical, educational and mental health providers.
• Conduct visits with children, parents and/or caregivers that include the on-going assessment of child safety, permanency, and well-being.
• Develop and implement a plan that meets the needs of the children and the family.
• Facilitate placement and delivery of services in collaboration with biological, foster and adoptive families.
• Perform case management responsibilities in accordance with state and federal laws, Department of Children and Families Administrative Codes and Operating Procedures, funding source policies and internal policies on child abuse, abandonment, and neglect within required timeframes.
• Attend court hearings, prepare and file all court documents within required timeframes.
