Job summary

McKesson Canada is seeking bilingual field case managers to join a growing Patient Support Program (PSP) supporting patients living with chronic and complex conditions. In this field-based role, you will act as a trusted partner to patients, healthcare providers, clinics, and pharmacies throughout the treatment journey.

You will help patients gain timely access to therapy, navigate reimbursement processes, receive education and ongoing support, and remain engaged in their treatment plans.

Qualifications

• Bilingual in French and English, both written and spoken.
• PSP experience or similar patient-support background preferred.
• Postsecondary education or equivalent combination of education and experience.
• Ability to manage multiple priorities in a dynamic environment.
• Strong organizational, communication, and relationship-building skills.

Responsibilities

• Serve as the primary contact for patients enrolled in the Patient Support Program.
• Assist patient onboarding, enrollment, and access-to-treatment activities.
• Educate patients on disease state, treatment expectations, and medication administration.
• Identify barriers to access and assist with ongoing therapy.
• Document patient assessments to support continuity of care.
• Collaborate with reimbursement specialists, clinics, and health-care providers.
• Support access initiatives by gathering information for reimbursement.
• Monitor patient progress and elevate concerns as needed.
• Build strong relationships with physicians, nurses, clinic staff, pharmacists, and other stakeholders.
• Provide program updates to health-care providers in line with privacy requirements.
• Support clinic activities and enhance the provider experience.
• Maintain accurate documentation within program systems.
• Participate in program meetings and process-improvement activities.
• Ensure activities comply with policies, procedures, and regulatory requirements.
