Position Summary

Center For Family Services is seeking Spanish-speaking,experienced, professional Family Support Coordinator for the ReConnect FamilySupport Program. The Family Support Coordinator position requires patience,dedication, understanding, and the ability to provide peer to peercompassionate support to program participants. The Spanish-speakingFamily Support Coordinator will cover 8 counties in Southern NJ.

Duties and Responsibilities:

  • 1:1 peer support service; in-person, via phone and/or telehealth.
  • Constructs individualized wellness plans with participants that are culturally competent with relevant services and identifies goals with measurable objectives the family member or friend wishes to achieve.
  • Offering resources to families seeking information on substance use disorder and behavioral health treatment. Resources include, but are not limited to, accessing treatment information through REACH-NJ, NJ Connect, private insurance,
  • Provide outreach and coordination of services with all systems involved with the family (i.e. support community, treatment community, IME, and Opioid Overdose Recovery Program (OORP).
  • Inform family members about Naloxone trainings to help prevent overdose with a loved one(s) with opioid use disorder and how to obtain Naloxone.
  • Educate the families on the different pathways of recovery to support their loved one(s) with substance use disorder in obtaining their individual based recovery pathway.
  • Advocate on behalf of families.
  • Provide lists of 12 Step and Recovery Support Group Meeting locations and/or other Mutual Support Groups for families.
  • Active listening to a family member, allowing them to share their stores of

success, struggles, and barriers.

  • Guide families using the CRAFT Method (Community Reinforcement and Family

Training) in exploring effective coping strategies such as self-care, stress management, personal boundaries and more.

  • Facilitate psychoeducation and support group meetings as needed.
  • Offer supportive strategies on how to motivate their loved one(s) to enter and

stay in treatment and connect with recovery support services.

  • Teach positive communication skills and be able to engage in role-plays with participants to rehearse effective communication skills.
  • Participating in outreaching and community events as needed.
  • Create an engaging and respectful environment that recognizes, supports, and encourages participants as important collaborators in their loved one(s) care.
  • Inform and Educate families of "patient brokering" that may result in unethical practices during their time of need to seek immediate treatment for their loved ones.
  • Performing Rutgers Surveys and follow-ups for families that qualify.

REQUIREMENTS:

  • Must be fluent in Spanish.
  • High School Graduate/GED required
  • Experience: Has 2 or more years of experience working in the substance use field and/or lived-in experience.
  • Completes all required trainings and acquires CPRS (Certified Peer Recovery Specialist) certification.
  • Vehicle with current driver's license, insurance and registration.
  • Applicant must be flexible and willing to work some nights for client appointments and the occasional weekend for events as well as travel throughout Southern NJ as needed to meet with clients.
  • Federal and State regulations require that employees for this program are fully vaccinated.
