Responsibilities

• Identify health and social service needs of families and plan program activities.
• Develop IFPA with family goals, responsibilities, and timelines.
• Provide community resources information and referrals to families.
• Conduct home visits and encourage parent volunteerism.
• Facilitate meetings and trainings; report suspected abuse per procedures.
