Job summary

Frontier Direct Care, LLC is hiring Family Nurse Practitioners and Physician Assistants to serve as the primary clinicians for direct primary care patients in Mesquite and Duncanville, TX. This full-scope role focuses on continuity, preventive care, and chronic disease management with longer visit times.

Responsibilities include in-office procedures, chronic disease management, and coordinating patient care with a Medical Assistant.

Qualifications

• Bilingual (English/Spanish) preferred.
• Active Texas licensure as a PA or APRN-FNP required.
• 2+ years post-graduate experience in family practice/primary care.
• DEA registration active or obtainable before start.
• BLS certification (within 30 days) and proficiency with EMR/scheduling tools.

Responsibilities

• Provide longitudinal primary care across chronic disease management, preventive care, and acute visits.
• Perform in-office procedures including I&Ds, suturing, skin biopsies, and men’s/women’s health exams.
• Conduct visits in person, by phone, and via secure text/video, 30 minutes to 1 hour.
• Diagnose and manage chronic conditions with appropriate panel size and visit time.
• Coordinate daily patient care with Medical Assistant and document visits in the EHR.
• Maintain 24/7/365 on-call access via secure company line.
• Work under supervising physician per Texas Medical Board rules.
• Participate in weekly team meetings and 1:1s.
