Responsibilities

• Initiates contact with eligible patients to describe the role and begin assessment.
• Schedules and completes initial assessment and follow-up visits.
• Provides educational information in-person and over the phone.
• Documents activities in the study database with accuracy.
• Raises awareness of chronic diseases in the community and links clients to services.
• Interacts with participants to support goal attainment and coordinates care.
