Job summary

City of Frederick's Federally Qualified Health Center seeks a Case Manager, Community Health to provide comprehensive case management and care coordination for patients facing barriers to healthcare.

You will work with providers, other Housing and Human Services case managers, and community partners to address social determinants of health, such as housing instability, food insecurity, transportation, and access to medications.

Qualifications

• Two years of healthcare case management experience.
• Experience serving underserved populations is desirable.
• Bilingual in English and Spanish required.
• Familiarity with social determinants of health.

Responsibilities

• Conduct intake assessments to evaluate patients' physical, behavioral, and social needs.
• Develop individualized care plans with healthcare providers.
• Coordinate referrals to specialists and mental health services.
• Assist patients in navigating Medicaid and other programs.
• Educate patients on conditions and self-management.
• Address social determinants affecting access to care.
• Connect patients with housing, food, transportation resources.
• Maintain confidential case records per HIPAA regulations.
• Document interactions in the electronic health record (EHR).
• Report outcomes to supervisors and funding agencies.
