Job summary

Washington County is hiring a Community Health Worker to assist individuals in navigating health and social services, focusing on health and wellness for individuals, families, and the community. The role acts as a liaison between county services and communities to improve access and cultural competence.

Bi-cultural or bilingual candidates, especially with ties to the Spanish-speaking community, are encouraged to apply. Hours are 8:00 am - 4:30 pm, Monday to Friday, with potential overtime.

Qualifications

• High school diploma or GED with at least 3 years of experience in human services or related field.
• Or completion of 2 years of post-secondary education with at least two years of experience in a human services or related field.
• Or completion of Community Health Worker certificate with two years of experience in a human services or related field.
• Must have and maintain a valid class D driver’s license.

Responsibilities

• Identify county and community resources to provide information, outreach, and coordination of care.
• Build and maintain relationships with community members and organizations.
• Educate and promote wellness with culturally appropriate health information and social determinants.
• Document work accurately and timely.
• Lead community engagement meetings and represent Washington County.
• Perform other duties as assigned.
