Neko is redefining preventive care. We are seeking a Clinical Care Coordinator in New York to guide members through referrals and follow-ups, ensuring safe, efficient navigation between clinical teams and external providers.

This role demands empathy, clear communication, and meticulous coordination across complex care journeys. You will own cases end-to-end, coordinate insurance checks and prior authorizations, and document activities with care.

Qualifications

• 2–4+ years of experience in patient navigation, referral coordination, care management, or healthcare operations.
• Native or fully fluent in both English and Spanish, with excellent written and verbal communication skills in both languages.
• Strong understanding of US healthcare systems, including insurance eligibility and prior authorisation workflows.
• Experience with EHRs or care coordination platforms (e.g. Epic, Athena, Elation, Salesforce Health Cloud, or similar).
• Demonstrated ability to manage sensitive member situations with empathy, professionalism, and sound judgement.
• Strong organisational skills and meticulous attention to detail across complex workflows.
• Excellent written and verbal communication skills with clinical and non-clinical stakeholders.
• Comfortable working in fast-paced, evolving environments with changing priorities.

Responsibilities

• Own individual member cases end-to-end, coordinating referrals and follow-ups with clinical teams and external providers.
• Coordinate insurance eligibility checks, prior authorisation requests, and specialist referrals within defined timelines.
• Maintain case documentation and ensure referral loops are closed with high quality,”
• Escalate complex or sensitive member situations with sound judgment to ensure safe, empathetic outcomes.
